Waist-to-hip ratio is a simple indicator of where body fat is concentrated — fat carried around the waist ('apple' shape) is associated with higher cardiovascular risk than fat carried at the hips ('pear' shape) at the same total body weight, which is why the WHO tracks this ratio alongside BMI rather than as a replacement for it.
Example: A waist of 85cm and hip of 100cm gives a ratio of 0.85 — for a woman, that falls right at the boundary between the WHO's "moderate risk" and "high risk" bands, while the same 0.85 ratio would read as "low risk" for a man, since the thresholds differ by gender.
